Nook Documents for Librarians

Here is where I post all of the forms, documents, and resources I find, modify, use, and create to implement our elementary school’s Nook pilot program.  All documents are under licensed under the Creative Commons Attribution-Noncommercial-Share Alike 3.0 United States License (see below), unless otherwise noted.

Parent and Student Permission Form 2011-12 (downloaded and modified from Buffy Hamilton & Kathy Parker)

Quick Start Guide / Troubleshooting for inside Nook covers, folds in half to fit in cover pockets

Nook mock covers for VHS boxes (to store on shelf as placeholders)
Nook bookmarks for inside Nook covers, barcodes can stick on the back for check out (downloaded and modified from Buffy Hamilton)
MARC title record of Nook Simple Touch Ereader (A new window will take you to the Edukindle Ning, where I posted the MARC record to download.)
Nooks #1-6 Tracking Form (downloaded and modified from Buffy Hamilton); I use different colors for each set of 6 and put these sheets in a binder for students to browse.

2011-12 Student Survey for inside Nook cover pockets
Gift Card Blank Record form for keeping track of each book we order with which card.  This might seem like overkill as BN.com has a history of purchases in your online account.  I also downloaded and modified this form from Buffy Hamilton.
COMING SOON! Shelf Marker Placeholders for Ebooks – These 2″ x 4″ stickers fit onto shelf markers or “book lookers,” which can help advertise to students what ebooks are available on the Nooks.

Documents I created, but decided not to use
Nook titles list for mock cases to slip inside into the back of the VHS boxes – I decided this was too much work, so I would just update the tracking form (see above) and put them in a binder for student use.
